Our Continuum of Care:

The Step by Step Community-Based Program serves individuals who are living in their homes, apartments, dorm rooms, or with family and roommates. All of the components that make up the Residential Program are available to community- based individuals who meet this level of care and are seeking adapted supports that cater to their individual needs. Individuals can admit directly into our community-based program if clinically indicated.

Our Clinical Care Model:

Clinical Care Support begins with an Individual Support Plan (ISP), developed in collaboration with and tailored to meet each individual’s unique needs and goals. Our clinicians work closely with each individual’s family and outpatient team to provide comprehensive and holistic care.

Clinical care meetings throughout each week address an individual’s goals and may include symptom management, executive functioning, coaching, educational and vocational support, whole-body wellness, assistance with obtaining and maintaining benefits, increasing socialization and peer connection, apartment maintenance and financial planning. Clinicians also support individuals with finding and engaging in productive and meaningful activities, which may include employment, volunteering, enrolling in courses at a local college or university, enrolling in adult education classes, and engaging in social groups (support groups, sports leagues, etc.). Community integration is often a focal point in case management as we encourage each individual to cultivate social networks within the larger community to work towards their goals.

Who is the Right Fit for Step by Step?:

We specialize in working with adults 21 and older who have a primary psychiatric diagnosis. We have found that individuals who thrive within our program demonstrate the below characteristics:

  • Willing and motivated to engage in treatment and develop independent living skills. We are a voluntary treatment program. If someone is not motivated and willing to engage in treatment, we may not be the best fit.

  • Has first received more specialized care to treat a specific mental health diagnosis, such as substance use, OCD or eating disorders.

  • Has demonstrated acute symptom stabilization by use of consistent clinical supports, including medication management and compliance and weekly outpatient therapy.

  • Can safely live independently and does not require overnight support.

  • Able to attend and uphold all programming requirements.

  • Have an IQ of 85 and above.
